Completion of All Forms Required to be Seen
All seven (7) of the forms are required to be seen for the intake session. You may be asked to reschedule if there are any missing forms not signed. This includes Form 7 The Intake.
Goodness of Fit
The first session is exploratory for both the client(s) and the therapist, and if there is not a good fit a referral may be made. This is for the client(s) to receive the best care.
Texting, Emailing, and Portal Instant Message
Texting is limited to scheduling information and questions over information that is not Protected Health Information protected by HIPPA. Please do not send Protected Health Information via email as it is not HIPPA secure. The Instant Messenger in the portal should be used for sharing brief Protected Health Information. Limit to one paragraph for it to be visible for the therapist.
Final Wrap Up Session
When therapy has reached the highest purpose (Aim) for seeking professional help there is a natural process of review and reflection known as the final session or Wrap Up. This is a natural progression and culmination of the work. Once a client always a client, and if ever there is a future need please don’t hesitate to call or text for services.
Text Link and CC on File
The session fee is collected through Clearent and a virtual terminal texted to the client. The cancellation fee of 50% of the total session charge, is collected by the CC on file. Please keep your CC on file up to date. Do not pay the pay link if you cancel less than two (2) business days prior to session (particularly day-of cancellations). The CC on file will be charged.
